3.6.2 Contractor shall submit CV for PMT to Company prior commencement Work. Company at its own
discretion may review and approve Contractor’s PMT team.
3.6.3 Any replacement of Site Manager and Coordinator during the Agreement Term requires approval
from the Company.
3.6.4 Company will apply penalty for the absence of each of PMT’s member for more than 14 days. The
penalty amount will be 0.1% of the invoice amount for that month per day. The penalty will be
counted starting the 15th day.
3.6.5 Contractor shall supply personnel, facilities and transportation as identified in scope of Work both
in qualification and quantity. These personnel shall be stationed at the Site on 14/14 rotation
schedule all the time or other rotation schedule as approved by Company. The cost incurred shall
be included in all unit rate.
3.6.6 Contractor Representative at minimum Site Manager level shall attend monthly meeting with
Company at Site, review Contractor progress, quality and HSE performance.
3.6.7 The qualification and responsibility for Contractor’s PMT shall be as follow:

Personnel Qualification Responsibilities

Site Manager • Minimum S1 degree; • Manage overall contract and

personnel including subcontractor
• Minimum 7 years experience in Pipeline personnel (if any);
inspection & maintenance or oil and gas
industry; • Provide monthly HSE Key
Performance Indicator (KPI) report;
• Experience in at least 2 projects in
operating gas or oil production facilities; • Accountable for approved HSE Plan
implementation;
• Having previous experience in contract
budgetary and resource allocation. • Monitor and maintain KPI and also
coordinate with Company’s HSE
personnel;
• Participate in Company’s HSE event
and program;
• Provide progress report, accrual and
invoice;
• Monitor Contract cost spending and
provide Contract monthly report;
• Manage communication with
Company.
• Accountable for smooth contract
implementation.

Coordinator • Minimum senior high school graduate;
• Minimum 5 years experience in Pipeline
inspection & maintenance;
• Having previous experience in pipeline
inspection & maintenance;
• Extensive familiarity with computer,
spreadsheet, application and scheduling.
• Coordinate with Company
Representatives regarding support
and activities required;
• Manage all team leader and crew,
including monitoring crew’s
attendance and disciplinary;
• Provide activity report (daily, weekly,
monthly), HSE report and daily
finding report (encroachment, pipe
expose, vandalism, thief, etc) to
Company;
• Responsible to implement approved
HSE Plan as per guidance from Site
Manager;
• Coordinate with Company
Representative regarding 30 days
plan development and progress; and
provide cost proposal;
• Provide HSE monitoring data, such
as MCU, HSE orientation, crew’s
contract validity, HSE matrix, etc; and
shall submit to Company monthly;
• Submit iCare, near-miss and hazard
hunting report to Company;
• Provide Permit to Work (PTW) prior
job execution.
