Requiring the services of PVBS Construction Services & Consultancy as Design

Consultant for the above-mentioned subject of the following project information below:

C. Total Area : 9,237 SQ.M

II. SCOPE OF WORKS

A. Preparation of Structural As-Built Plan

B. Signing and Sealing of Documents

C. Submission of Standard and Specifications

III. PROPOSED WORK SCHEDULE

The proposed work schedule to carry-out the work will start provided that the Client
conform on the proposal and issue the “Notice to Proceed” to commence the work , to wit:

IV. PROFESSIONAL FEES AND EXPENSES

The DESIGN CONSULTANT is expected to provide a summary of fixed fees and

expenses, including taxes and other duties, for providing design consultancy services as
follows:

V. WORK DURATION

Time table to carry out all works shall be minimum of Thirty (30) days or more or less
Five Months upon approval /conforming this proposal and shall be extended depends on
circumstances thereof.

VI. TERMS OF PAYMENT

The following terms of payment shall be as follows:

50% : Down payment

50% : Upon Completion of Work

VII. MISCELLANEOUS / EXTRA WORKS

Any additional works requested by the owner after approval of this proposal not included
on the program of works shall be charged as Extra Work Order with separate cost per
item.
