Construction Services to carry out the project to wit:

I. PROJECT DESCRIPTION

Requiring the services of PVBS Construction Services & Consultancy as a Contractor

for the above-mentioned subject of the following project information below:

II. SCOPE OF WORKS

A. Supply and installation of Concrete Trench Canal with Steel Grille Cover

B. Supply and installation of Concrete Oil Interceptor with Steel Plate Cover

C. Tapping of 2-Existing area drain to the Oil Interceptor

D. Tapping of Oil Interceptor to the Existing Sidewalk Drainage

III. PROPOSED WORK SCHEDULE

The proposed work schedule to carry-out the work and construction will start provided
that the Client conform on the proposal and issue the “Notice to Proceed” to commence
the work , to wit:

IV. SUMMARY OF PROPOSAL

PVBS Construction Services & Consultancy is expected to provide the summary of cost
including indirect cost and tax for providing Construction Services as follows:

VI. WORK DURATION

Time table to carry out all works shall be minimum of Forty (40) days or more or less
Six Months upon approval /conforming this proposal and shall be extended depends on
circumstances thereof.

VII. TERMS OF PAYMENT

The following terms of payment shall be as follows:

50% : Down payment

50% : Upon Completion of Work

VIII. MISCELLANEOUS / EXTRA WORKS

Any additional works requested by the owner after approval of this proposal not included
on the program of works shall be charged as Extra Work Order with separate cost per
item.
