AFFIDAVIT OF LOSS

I, GIANNI ROEM V. TUNAC, of legal age, Filipino, single, and a resident

of Brgy. 2 Vintar, Ilocos Norte, Philippines, after having been sworn to in
accordance with, hereby depose and state: THAT----------

1. I am a Holder of a Non-Professional License Identification (ID) Card by the

Land Transportation Office (LTO);

2. I always keep the aforementioned License inside my wallet, where I keep

all of my Identification Cards;

3. Sometime in February 23, 2018, when I reached home after going to the
school, I noticed that the aforementioned license was no longer in my wallet.
It might have slipped unnoticed when I was bringing out money to pay
something;

4. Immediately, I went back to the school and look for my license;

5. I exerted every effort in locating my license, but to no avail. Hence, my

license ID Card issued to me by the LTO is now deemed lost and beyond
recovery;

6. The aforementioned License ID Card was never been confiscated in violation

of existing law nor made as a security; and

7. I am executing this affidavit to attest the truth of the foregoing facts,

specifically the fact that license ID Card issued to me by the LTO is now
deemed lost and beyond recovery. This affidavit is also being executed for
whatever legal purpose it may serve.
