Nurse : “Hello, good morning Ms?”
Patient : ”Good morning”
Nurse : “I am Nurse Mike and I am shifted for registration.
what is your name?”
Patient : “My name is Mrs Rita, nurse”

Offering Help
Nurse : “Okay. Mrs Rita what can I do for you?”
Patient : “I have a problem with my stomach”
Nurse : “oh I see, sit down please.”
Patient : “yeah thank you nurse”
Nurse : “What is a problem with your stomach? What is your complaint Mrs? ”
Patient : “Yeah, I have a stomach pain and nausea”
Nurse : “Oh I see, and then Do you want to check your stomach?”
Patient : “Yes, I do nurse”

Nurse : “Okay Mrs. But before you check your stomach you must doing registration in here.”
Patient : “Okay”

Mrs. Anne : “I think I have no serious problem but, I want to know my condition exactly.”
Nurse Dewi : “I appreciate the awareness of health you have. Have you come here before?”
Mrs. Anne : “It is the first time I come to this hospital.”
Nurse Dewi : “Alright. We always provide the best service to our patient. What Kind of medical check-up
do you want?”
Mrs. Anne : “I don’t know, can you help me to choose?”
Nurse Dewi : “Sure. We Have many kind of medical check-up. They are basic package, standard
package, essential package, and executive package. For women, we have additional examination. I
recommend to you to choose executive package, it has complete examination, and the additional
examination is pap-smear, to check the existence of cervical cancer.
Mrs. Anne : “Can you mention the services?”
Nurse Dewi : “You will get the physical examination from specialist in internal medicine, eyes
examination, thorax X-ray, ENT examination, ECG and treadmill test, hearing test, lung function test,
laboratory test for blood examination, urine examination, Lipid profile, hepatitis test, renal function and liver
function. You also get a free lunch.”

1980, you are an executive employee, you choose executive package, and your internist is dr. Hanna
Vania, Sp.PD, is that right?”
Mrs. Anne : “Yes, right.”
Nurse Dewi : “I will input your data to the computer. You can wait in the waiting room, in front of this
admission department, until your name called by nurse again. Is there anything I can help again?”
Mrs. Anne : “How long the examination will run? What should I do after this?"
Nurse Dewi : “Usually about five to seven hours. You will give an ID bracelet by the nurse, then you
should change your clothes with patient’s shirt and do the examination. If you have any question while

Situation: The following dialogue between a nurse and a patient who’s just
being admitted to hospital. The nurse is taking personal details and asking
about the patient’s general state of health.

Nurse : Good morning Ms. Natasha

My name‘s Gita, I’m a registered nurse, and I’ll be looking after you today.
How are you feeling now?

Patient : Good morning. I’m feeling alright just now, thank you.
But I’m feeling a little nervous about being in hospital.

Nurse : Don’t worry, there’s nothing for you to be concerned about.

Now I’ve got several questions I need to ask you, and some paperwork to go
through, for your admission. Are you ready?

Patient : Yes, I suppose so. What do you need to ask me?

Nurse : Firstly, I’d like to check your personal details.

Yours surname is Pradita Natasha Is that correct?
 And are you married ?

Patient : Yes, I’m married. Can you see how you’ve written my name?
Thank you. Yes, that’s correct.

Nurse : Are you still living at the same address? Harapan Indah 2 block D no. 8 ?

Patient : Yes, I still live at that address. I was born November 13, 1965.
And I’m Moslem

Nurse : Can you give me a few details about your medical history?
Have you or any close relatives ever had hypertension,
diabetes,
asthma, heart disease, cancer, or any operation?

Patient : I had high blood pressure 7 years ago, and nearly two years
ago
I was diagnosed with diabetes. I’m controlling for high blood pressure very well
with diet and exercise, but I can’t controlling for my diabetes mellitus.

I also had a heart attack 5 years ago. I had to have a heart

operation.

Nurse : And what’s the reason you’ve come into the hospital today?

Patient : When I was walking to the shops to buy the paper this
morning,
I suddenly got a tight feeling in headache, reduced visibility, easy weak, easily
hungry, thirsty easily.

I thought it was a good idea to come to the hospital.

Nurse : Are you taking any medications?

And are you allergic to any drugs or foods?
And do you smoke or drink?

Patient : I’m only taking some vitamins, and as far as I know, I’m not allergic
to anything.

I gave up smoking 5 years ago, after my heart operation, and we

only drink a little wine on special occasions.
 Nurse : Are you in any private health fund?

Patient : No, we’re not in any health fund.

Nurse : I’ll just take your temperature, blood pressure, blood sugar, physical
examination like weight, blood sample, and others. And then get you to write
down the phone number of your next of kin.

The doctor will come and see you in a few minutes.

Patient : Thank you. I hope I won’t be here very long. I don’t like being

hospital.
