Job Summary:
The post holder will be an experienced Paramedic who, acting within their scope of practice, will provide
clinical assessment and management of patients who are triaged and identified as requiring an out of hours
home visit.
The post holder will carry out visits to patient’s private homes, as well as possible visits to patients in Nursing
/ Care homes.
Supported by a multi-disciplinary team, they will work within the community, as an autonomous,
accountable, Paramedic Practitioner, in the provision of a holistic approach for patients including
assessment, diagnosis and treatment, to deliver quality patient services.
Committed to the development of innovative practice and expanding evidence-based care which will
contribute to the enhancement of the Paramedic Practitioner role.
Provide and maintain a high standard of care for patients in their own homes as well as providing support
to the GP’s and other members of the primary health care team.

Key Responsibility Areas:
Clinical
Making an assessment of their physical, psychological and social needs through focused history
taking
Develop a clear management plan with patients and referral / admission
Know own limits in relation to scope of practice and recognise when a requirement exists to refer
to, or seek advice from a more senior clinician.
Adopt an evidence-based approach to patient care
Have a broad range of knowledge, clinical skills and competencies to enable the needs of patients
to be met
Maintain anaphylaxis and resuscitation techniques within scope of practice
Recognise the signs, symptoms and categories of child abuse. Have an understanding of child health
procedures, statutory local procedures and points of referral and the basic legal issues around child
safeguarding
Recognise and be aware of appropriate referral mechanisms for family violence, vulnerable adults
and addictive behaviours
Recognise psychological needs of patients with depression or suicidal tendency and respond
appropriately to refer to support teams/agencies as required
Professional
Provide a high standard of professional conduct and care at all times in accordance with the HCPC
requirements
Attend relevant training and education to maintain professional and clinical competence
Adhere to the Medicines Act of 1968 and the HCPC Guidelines for the administration of medicines
Work within own professional competence at all times and refer to Duty Doctor / GP as necessary
Ensure accurate record keeping of all consultations are maintained at all times
Undertake an active role in audit that will have the broad objective of improving or re-evaluating
this service
Abide by the National Health Service (NHS) Code of Conduct
Perform any other duties as may from time to time be reasonably required
Attend mandatory training
To ensure delivering of high quality and standard care service required by HCPC, postholder is expected to
comply with all clinical policies, protocols, procedures and specific training on:
