JUDICIAL AFFIDAVIT

I, DR. SACHA MAY CHAVEZ, single, of legal age,

residing at Sor D Rubio St. Bogo City, Cebu, answer the
following questions propounded by Atty. Gwen Stefani
with office address at 28th floor, TGU Building, Cebu IT
Park, Cebu City. The same address where the examination
was held.

1.Q. How long have you been practicing your

profession?
A. For almost nine (9) years already.

2.Q. Where are you presently employed?

A. I am a senior medical examiner in Chong Hua
Hospital.

3.Q. Have you ever examined a woman named Micyle

Rodriguez?
A. Yes. Her dead body was sent to the hospital for
examination. I believe I talked to her parents,
they wanted to do a separate and independent
examination of the body to confirm how she died,
as the medical results of Verallo Medical
Hospital conclude she died from a heart attack.

4.Q. Have you seen the documents showing the medical

findings of Verallo Medical Hospital(Exhibit “A”)?
A. Yes. I was confused at first because when we did
our examination there was no showing that Ms.
Rodriguez died of a heart attack.

5.Q. Did you sign this medical record (exhibit “B”)?

 A. Yes I did sign that medical record. The contents
of that medical record (exhibit “B”) shows our
findings that Ms. Rodriguez died of asphyxiation
or death by lack of oxygen.

6.Q. How can you be certain that your findings are

accurate?
A. I have been in the medical field for nine (9)
years as a medical examiner. Her cause of death
is not uncommon and can be ascertained with
accuracy.

7.Q. Do you have any other statements to add?

A. No. That would be all

exhibits and referred to in the foregoing answers are
true, correct and faithful reproductions of the
original.
