SUBJECT : Initial Report on Fire Incident

1. Reference: Initial Report on Fire Incident from Pagadian CPS dated 03 April
2019.

2. Please be informed that on April 3, 2019 at about 12:05 PM, a fire incident
transpired at Chicken Deli Restaurant located at F.S. Pajares Avenue, Barangay Gatas,
Pagadian City, Zamboanga del Sur wherein the stock room of said restaurant was razed
by fire.

3. Initial investigation disclosed that the fire started at the stock room as
witnessed by one of the employees of said restaurant.

4. The fire out was declared around 12:20 PM of same date by the BFP
Pagadian City with no casualty, while the estimated cost of damaged is yet to be
determined.
