Greetings. I write today to your kind office to formally convey my request to avail of the refund
offer under my account with R.A. No. PHL- 2015-00095-1 under my name for the property
located at Block 12, Lot 3, Pasadena Homes, City of Binan, Laguna.

The total amount that I claim for reimbursement is Seventy Three Thousand Six Hundred
Seventy Nine Pesos & 94/100 (Php 73,679.94). Please see the attached Equity-Payment
Schedule for your reference.
