Reminders: This worksheet is intended to absentee with valid reason(s). If the activities will not be able to submit by
the absentee despite of all reminders on the scheduled date, student is considered failed in the particular activity.
Excused letter (signed by guardian) is needed as proof of his/her reason. Absent student’s score is not the same as the
student who attended and did the activity during class hours. This will not suffice the performance task which is 60%
of the grades of the student. Students must attend the class at all times. Write your answer in one whole sheet of
paper.
