AFFIDAVIT OF SERVICE

I, REX C. SAGAUINIT, male, Filipino, single, of legal age, with office

address located at Unit 33L Pearl of the Orient Tower, 1240 Roxas Blvd., Ermita,
Manila 1000, after having been duly sworn in accordance with law, hereby depose
and state: THAT-

1. On August 8, 2019, this Honorable Court issued a SUBPOENA

AD TESTIFICANDUM, a portion of which states:
 “Respectfully referred to Atty. Rex Sagauinit, Private Prosecutor, the attached
SUBPOENA dated August 8, 2019 for service of the person(s) named therein
and FOR THE RETURN OF SERVICE THEREOF, WITHIN
FIVE (5) DAYS FROM SERVICE.”

2. Pursuant to the above-quoted Order, I diligently searched for the

whereabouts of the private complainants and communicated the matter to them.
On August 9, 2019 at around 3 P.M., I personally served the subpoena to private
complainant ,JUREM SABAD, who happened to be in manila and was just
about to travel back to North Cotabato on the same day. (Attached as Annex “A”
is a copy of a picture showing that he received the subpoena and made an integral part hereof)

3. On August 12, 2019, I travelled to Cotabato City where I

personally served the subpoena to other private complainants, namely, ALLAN
ONDO CATULONG, JOEMAR ANTIPUESTO, ALBERT
ANTIPUESTO, EPHRAIM TIZON, JEFFREY TABEROS and witness
GLORY VISAYAS (as represented by his sister Eldie Visayas Porsado with
copy of her valid ID , who personally received the subpoena as Glory was not
around at the time) at around 11 A.M. (Attached as Annexes “B” to “G” are copies
of pictures showing that they received the subpoena and made an integral part hereof)

4. As to private complainant JUNWARD LAROSCAIN, the

subpoena was personally served to his sister named Edlyn Laroscain Lubay as
Junward was not around at the time at around 4 P.M. (Attached as Annex “H” is
a copy of a picture showing that she received the subpoena and made an integral part hereof)

5. At the said place and on or about the said time, I personally met
the above-named private complainants and witness, and upon explaining to them
the purpose of the said subpoena, caused the service, as I actually served, to them
copies of the above-stated SUBPOEN AD TESTIFICANDUM of this
Honorable Court by asking them to receive it and by handing them copies
thereof.

6. The abovenamed private complainants and witness, as proof of

their receipt, personally signed their names and affixed their respective thumb
marks on the face of the subject Subpoena. . (Attached as Annex “I” is the receiving
copy of the Subpoena and made an integral part hereof)

Attached as Annex “J” and “K” are copies of my back and forth plane tickets and made
an integral part hereof

7. Thus, the Order of this Honorable Court has been sufficiently and
duly served.

IN WITNESS WHEREOF, I have hereto signed my name this 13th day
