SUBJECT: NOTICE OF TERMINATION

Dear Sir,

We are writing this letter to formally inform your good office that we are terminating our
partnership with your company due to the following reason/s:

1. Non- Payment of your overdue payables to us which is FORTY ONE THOUSAND EIGHT
HUNDRED EIGHT and 14/100 PESOS (P41,808.14) representing the value of the Services
Rendered for the period of June 16, 2018 to July 31, 2018 which are already due and
demandable. Time and again, we never fail to remind you to pay and settle your outstanding
obligation in the company.

2. We have not received a signed service agreement nor a commitment from you as to when we
can secure a copy to manifest your intention to renew the said contract. Further, the notarized
service agreement will be submitted to DOLE for compliance and enforcement of labor laws.

Accordingly, since you fail to pay your outstanding balance from us, you left us with no choice
but to put an end to our relationship as partner-clients effective Thirty (30) days upon receipt of
this letter, unless you are willing to settle and submit the signed service agreement .Hence, we
should hold you to be liable for the non-payment of your outstanding obligation to the company.
