SUBJECT: ACCEPTANCE LETTER FOR SCHOLARSHIP

Dear Jay John:

On behalf of ASA Philippines Foundation Inc., I am pleased to inform you that you
have been chosen to receive ASA Philippines Scholarship in the amount of P7,500
scholarship grant per semester or a total of P15,000 annually for your studies.

I. Forfeiture of Scholarship: The scholar understands the award could be


rescinded in any of the following circumstances:
1.) Failure to maintain the required minimum WPA of 80% for 2
consecutive semesters.
2.) Dropping, shifting of course, and transferring of school is subject
for approval of the management. Dropping of subject must have
valid reasons which are stated below:
i. Conflict of schedule
ii. Subject was closed or dissolved
3.) Involvement in infraction of school policies.
4.) Involvement in illegal and immoral acts.
5.) Having vices like gambling, smoking and alcoholic drinking.
6.) Having an intimate relationship with MFO or any employee of the
Foundation.
7.) Mother exit from the program or become delinquent. In cases, the
scholar has shown extra special academic performance, approval
from the management can be sought upon recommendation from
the operations.

II. Deferment of Scholarship:

A Scholar who wants to defer his/her scholarship must contact the ASA
Philippines staff in writing stating why he/she wish to defer and when
he/she intends to enroll. ASA Philippines staff may request additional
information or proof of extenuating circumstances and reserve the right to
deny the request, in which case the awardee forfeits the scholarship.
III. Accountability in Case of Drop-Out

The grantee / scholar shall reimburse to the Foundation the cash amount
received for the semester or it would be converted to the loan of the client
in case of drop out or failed to finish the semester due to willful
abandonment of his/her study.
