2019 Educational Scholarship Award


Application

The recipient of this award must:


• Be a graduating senior of OHS
• Plan to attend a two or four-year college or university, a vocational/technical program, or a
certificate program for the 2019/2020 school year

Application Requirements
The applicant must submit the following no later than Friday, April 1, 2019:
• A completed application, legibly written in ink
• An original personal statement (350-word minimum), which discusses the student’s expectations and
aspirations for college, or selected post-secondary education and beyond. This essay should be used to
explain why attending college or the selected program is important to the student. This may be
handwritten or typed.
• One teacher or guidance counselor recommendation (page 7) to be returned directly by the
recommender to the guidance office
• A completed OHS Guidance Office Reference Form (page 2)

You will be asked to supply your student ID number instead of your name to keep all
information confidential. Only the guidance office will have access to names and addresses
of applicants.
