Vessel at Anchorage Campha, Vietnam and cargo operation in progress.


Around 1900H LT 04th Feb. 2020, during loading operation deck crew found damage (multiple cut
wire strand) in Hoisting wire of Deck Crane No. 1. Record to date of Hoisting wire Total Running
Hours 487 Hrs. Crew renew the wire at 1930H LT and completed replacement at 2345H LT.

Replace new one of Ship Crane No. 1 Hoisting Wire

Guideline for DAMAGE REPORT

When trouble/damage occurs to a machinery or device, the Master shall report to the SI using this
form (in accordance with FMS0702 Section 4 and QSMS Manual Chapter 11. “Deficiency and Risk
Management Regulations”.)

1. When repair is completed on board,


The master shall fill in column 1 to 4 and checks box in column 5 (Not necessary).
5. Shore based support:
☐ Request ☒ Not necessary (completed repairing on board)

Then submit to the SI with signature.


(This initial report (copy) shall be filed individually for each damage.)

SI shall review the master’s report and fill in column 6-8 and closing date, then send back to the ship
with signature.
