ON THE JOB TRAINING / INTERNSHIP GUIDELINES

OBJECTIVES
This guide aims to assist students relative to the conduct of their On the Job Training Program/
Internship from applications to completion. Specifically, the training aims to:
1. Provide students with opportunities to apply relevant knowledge and skills acquired
from formal education to actual work settings provided by reputable Host Training
Establishment (HTEs) in the country;
2. Enhance the knowledge and skills acquired in formal education of student interns
through employer – based training, in order for them to become more responsive
to the future demands of the labor market;
3. Develop life skills of student interns, including those relevant to the values of
professionalism and work appreciation;
4. Provide platform for the students to acquire soft skills necessary to address the
demand of the employers such as communication skills, interpersonal skills,
financial literacy, etc.; and
5. Provide an opportunity for the students to acquire a professional work ethics in the
course of internship.

1 Student Coordinate with registration adviser for evaluation of grade
2 Student Attend the Orientation for On the Job Training Program
3 Student Apply for On the Job Training (secure CEIT OJT form 1)
4 OJT Coordinator Evaluates if the student qualifies for OJT
5 Student Enroll OJT subject
6 Student Submit OJT requirements to the OJT Coordinator
7 OJT Coordinator Prepare recommendation/ endorsement letter
8 Student Submit recommendation/ endorsement letter to HTEs
9 Company/ HTEs Accept / Process request
Note: if accepted, return acceptance letter to the student
10 Student Submit signed acceptance letter to OJT coordinator
11 OJT Coordinator Prepare Memorandum of Agreement (MOA)
12 Student Submit MOA to HTEs for signature
13 Student Submit signed MOA to OJT Coordinator
14 OJT Coordinator Monitor student performance through site visitation, phone call,
online follow up, or periodic meetings in the college
15 Student Attends meeting with coordinator
16 Student Completes the training
17 Company/ HTEs Issues Certificate of Completion and accomplishes Student
Performance Evaluation Form
18 Student Submit all documents and requirements to assign OJT Coordinator
19 OJT Coordinator Evaluates the documents submitted by trainee and submit student
grade report

Academic Performance 40 Attendance and compliance with documentary requirement

b. Daily Work Journal (20%)
c. OJT Narrative Report (50%)

OFFENCES AND SANCTIONS
1. Verbal Warning
2. Rectify the situation through a written apology. Written warning will be issue
3. Termination from OJT

OFFENCES SANCTION
Transfer to another host establishment without permission or approval from 1 2 3

Absences without prior notice to the immediate supervisor 1 2 3
Accumulated. Unjustified absences, or habitual tardiness 1 2 3
Acts of disrespect in words, gestures, signs or deeds which ridicule any 1 2 3
coordinator, company personnel and/or fellow OJT
Playing computer games or watching movies while on duty 1 2 3

Making personal arrangements/favors with the company or its representative 2 3
Refusal to cooperate with persons in authority 2 3
Posting defamatory comments or statements directed against the University, any 2 3
of its units, and /or any member of the academic community as well as the host
training establishment and its personnel
Direct assault or attempt to inflict bodily injury to coordinator and or any personnel 3
of the host training establishment
Romantic or sexual involvement with any office personnel or fellow OJT students 3
Misuse of evaluation form or tampering official record or documents 3
Vandalism, destruction, and/or stealing property or equipment 3
Accessing a record or file without consent of authority 3
Disclosure of confidential information 3

CONFIDENTIALITY
I understand that all the information on technology, manufacturing process, process
standards, quality assurance, methodologies, quality standards, production capabilities, raw
material purchasing, marketing finance, and all other related documents, manuals, operational
and technical matters that the Host Company shall make available to me, shall considered as
Confidential Information.

full responsibility for the knowledge and observance of all procedures, policies and
regulations.
