NARRATIVE SELF-EVALUATION OF THE INTERNSHIP EXPERIENCE


I had my 300 hours internship training at Zambales Rural Bank in San Marcelino, Zambales. On
the job training is a training that will help an individual to enhance and improve their skills that
can use in their future job.

The Zambales Rural Bank is the nearest and most convenient I can think of to have an internship.
It took me weeks before I started my internship there. Weeks later, Zambales Rural Bank have
sent a message saying that they would want to have me as an intern in their San Marcelino branch.

In my first day as an intern, the employees, as well as the manager, approached me and introduced
myself and they introduced themselves as well. I observe a lot on their daily routines, works, and
other stuff to familiarize myself on their work environment. I was kind of nervous and excited at
the same time to meet new people and to work with them. They guided and told me everything I
had to know. During my OJT in Zambales Rural Bank, I was assigned to do run-up the everyday
transaction, encoding of the new accounts, sorting files, sizing of accounts, and stamping the date
on the receipts. That is my daily routine for almost four months of my internship in the bank.

I learned a lot of things during my OJT days, one of those things are being sociable and skillful.
Socializing with other people was one of the reasons I became confident talking to other people
and make friends. It wasn’t easy at first to be friends with the employees because I was nervous
and shy but luckily they were very approachable and didn’t make us feel like we are out casted so
we got along well in just few days. I felt good about how the employees as well as the area head
of the bank treated us because they were kind and mindful in a way that they were willing to teach
us everything we had to know. We became friends and Zambales Rural Bank gave me an
opportunity to enhance my communication skill to deal with different clients and its helps me to
be more confident and also they gave me an idea about how the real business world works.

I would like to express my gratitude to Columban College for giving us an opportunity to see the
real world. It helps us in building ourselves to become a more matured in dealing with a lot of
things. It was really a big help and made us realize things. And to our very own College Dean, Mr.
Eric Matriano and to our Chairperson Mr. Peter O. Tulio, I would like to thank them for guiding
us and for supporting us in this journey of every graduating student. I make sure that I will apply
all my learnings to my future job and always be thankful to the Columban College School.

BRIEF HISTORY OF ZAMBALES RURAL BANK

Zambales Rural Bank, also known as Zambank, is based on Olongapo City and has
branches across Zambales, Bataan and Pampanga. It started its’ humble beginnings in 1970 and
the enterprise operates in the Commercial Banking Industry. Today it has 12 operating branches.
The bank also prides itself as one of the biggest lender to individual palay and animal farmers in
Zambales.
The Zambales Rural Bank was took over the management by the Coloma Family in 1995.
They controlled the 98% of the management on 12 branches across Zambales, Bataan, and
Pampanga and has resources worth P1.3 billion. Jerry B. Coloma Jr., CPA, graduated at University
of the East with a degree in Bachelor of Science in Business Administration, and has 30 years of
rural banking experience, while his wife, Mercedes Santos Coloma, CPA, also graduated at
University of the East with a degree in Bachelor of Science in Business Administration, and has
45 years of rural banking experience. She also awarded as Rural Banker of the Year in 1990. The
Coloma family purchased the Zambales Rural Bank in 1996 at par value from stockholders with
resources of P16 million, has negative net worth of P2 million, and past due ratio of 80% when
they bought it. They got the majority shares then managed it well. Right now, the Zambales Rural
Bank has a resources of P1.3 billion, net worth of P140 million, and past due of 9.9% under the
management and control by the Coloma family.

MEMORANDUM OF AGREEMENT
KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement made and executed, by and between:

COLUMBAN COLLEGE, INC., a non-stock, non-profit educational institution, duly organized


and existing under Philippine Laws with office/business address at No. 1 First Street, East
Tapinac, Olongapo City represented herein by DR. ERIC AGULLANA MATRIANO, Dean,
College of Business and Accountancy, hereinafter referred to as the COLLEGE.

-and-

ZAMBALES RURAL BANK, a business enterprise duly organized and existing under the laws
of the Philippines, with principal office/business address at San Marcelino Terraces Bldg., Brgy.
Central, San Marcelino, Zambales represented herein by MS. REMBEL M. VIERNES,
BRANCH MANAGER herein after referred to as the COMPANY.

WITHNESSETH:

Whereas, the COLLEGE will provide on-the-job student trainees upon needed and requested by
the COMPANY.

Whereas, the COMPANY will provide practical and technical experiences related to but not
limited to accounting services and management, auditing, financial reporting, bookkeeping, and
the likes to the on-the-job student-trainees.

NOW THEREFORE, the parties hereby bind themselves to undertake a Memorandum of


Agreement for the purpose of supporting the COLLEGE's on-the-job training for student under
the following terms and conditions:

1. The COLLEGE shall be responsible for the recommendation of the student-trainees who
intend to conduct on-the-job training or practicum exposure in the COMPANY as part of
the COLLEGE's curriculum;

2. The COLLEGE shall provide the student-trainees the basic orientation on work ethics,
conduct and discipline to ensure smooth relationship and cooperation with the
COMPANY;

3. The COLLEGE shall issue an official signed letter of endorsement/recommendation


assuring the well-being of the student-trainee which shall be used by the COMPANY for
processing the application of the student-trainee;

4. The COLLEGE shall voluntarily withdraw a student-trainee who is found to misbehave


and/or act in defiance to existing standards, rules and regulations of the COMPANY and
impose necessary sanctions to the said student-trainee;

5. The COMPANY may grant allowance and/or adequate insurance to student-trainee in


accordance with the COMPANY's existing rules and regulations;

6. The COMPANY, upon consultation with the COLLEGE, may require qualified students
to submit themselves to examination, interviews, and file pertinent documents to support
their application;

7. The COMPANY shall allow the Practicum Adviser, the Program Chairperson, the Dean,
or any representative assigned by the COLLEGE to visit and assess the working
performance and attitude of the student-trainee;

8. The COMPANY is not obliged to employ student-trainee upon completion of the training
and it will be on the discretion of the COMPANY to absorb the student-trainee based on
his/her performance;

9. The COMPANY shall issue a certification to the student-trainee upon completion of the
training together with other pertinent documents required by the COLLEGE (like DTR
copy and signed OJT Evaluation Form);

10. The student-trainee shall be personally responsible for any and all liabilities arising from
negligence in the performance of his/her duties and functions while under training;

11. The student-trainee shall be personally responsible on collating and processing the required
pertinent documents that will be coming from the COMPANY (like photocopy of DTR,
certification, signed OJT Evaluation Form, and the likes;

12. The student-trainee shall submit a journal every end of the week as part of his/her Weekly
Accomplishment Report signed by his/her Immediate supervisor from the COMPANY to
monitor the performance of the student-trainee;

13. The student-trainee shall submit a portfolio upon completion of his/her on-the-job training
complete with pictures and narrative reports as part of the requirements of the COLLEGE
with affixed signature coming from his/her immediate supervisor;

14. There is no employer-employee relationship between the COMPANY and the student-
trainee;

15. The duration of the program shall be equivalent to 300 working hours unless otherwise
agreed upon by the COMPANY and the COLLEGE;

16. Any violation of the foregoing covenants will warrant the cancellation of the Memorandum
of Agreement by the COMPANY within 30 days upon notice to the COLLEGE;

17. This Memorandum of Agreement shall become effective upon signature of both parties
and implementation will begin immediately and shall continue to be valid thereafter until
written notice is given by either party, thirty (30) days prior to the date of intended
termination.

INTERNSHIP MANUAL

PROGRAM RATIONALE

The Internship Program supports students in their application of the internship as a


preparation for professional work as Accountants. The primary instructional strategies used are
self-directed learning, self-assessment, and reflective reasoning. It is purposeful, provide
opportunities for reflection, present a continual challenge to the student, and incorporate active
learning, with the student an active participant in all stages of the learning experience from
planning to evaluation. The students are trained by industry practitioners during their 300 hours
actual training in their chosen business industry.

The undergraduate internship is a requirement for those who are incoming Graduating
Students of the BSA/BSAT Program. It is a course of study and on of the major components within
the curriculum that will provide practical and technical experiences to the students related to
Accountancy.

This is an immersion program wherein the student-interns will have the chance and
opportunity to be with the industry. The program will apply the skills, knowledge and conduct of
the student in a real working environment of the theories learned and developed during academic
hours.

As a general rule, only those who have completed at least 90% the required subjects shall
be allowed to enroll in the course.

This document contains the general policy and guidelines for the internship programs of
students enrolled under the BSA/BSAT Program.

GENERAL OBJECTIVES

In line with the mission to offer dynamic programs of study to BSA/BSAT graduates
designed to meet the general and specialized needs of the business industry for the advantage of
the local and global economy the internship program aims to:

a. It will enable the student to apply and appreciate the relevance of classroom learning
to the actual work setting.

b. It will enable the student to gain experiences, relate and apply the theories learned in
the classroom to real situations and problems.

c. Accustomed with the daily activities and routine of the industry operations.

d. Provide the students with insight of the various processes, techniques and
troubleshooting technique in the real work setting.

e. Student will be able to appreciate the value of human relations, understanding


individual differences in an actual work environment.

f. Understand the demand of the industry in terms of time management, personality


development, and scope of the industry.

g. To conduct pre-orientation of students before deployment in the business industries


related to the different fields of accounting of their choice.

h. To send students for internship in the business industries related to the different fields
of accounting of their choice for exposure and preparation for professional work.

i. To conduct post conference on the evaluation of students performance.

GENERAL REQUIREMENTS

1. The BSA/BSAT student who finished all the required prior BSA/BSAT major subject of
the course may enroll for Internship.

2. The minimum required number of hours of internship for the BSA/BSAT program is 300
hours.

3. A student must submit curriculum vitae at the BSA/BSAT office upon enrolling the
Internship course

4. A student is required to submit all documents pertaining to their application and


acceptance for Internship.

5. The chairperson of the Accountancy program will be the Internship Coordinator.

6. Every student is required to submit a copy of daily time record, accomplishment report
by term, evaluation and certificate of training compiled in PORTFOLIO.

7. The student intern must report to the internship coordinator of his concerns.

8. The student intern will be evaluated by the immediate assigned supervisor of manager
from the business industry.

INTERNSHIP REQUIREMENTS

The following are the list of internship requirements to be submitted to the Coordinator:

 Initial Requirement for the Endorsement of the Faculty in charge to the business
industry, and duly approved/noted by the College Dean.

1. Comprehensive resume
2. Students must be enrolled in the subject.
3. Compilation of Certificates from different seminar or workshops attended.
4. Letter of Endorsement

 Final Portfolio Requirement – To be submitted to the Coordinator for proper


evaluation of the student’s performance.

1. A brief history of the business industry.


2. Daily activities in a narrative report.
3. Signed internship Letter of Acceptance
4. Memorandum of Agreement (MOA) contact signed by the business industry’s
representative, students and faculty in charge.
5. Policies/guidelines issued and signed by the representative of the business industry.
6. Certificate of Internship (300 hours).
7. Original Internship Evaluation Report duly signed by the immediate supervisor
8. Original copy of the daily time record duly signed by the HRD of the business
industry.
9. Student activity record signed by the supervisor.

GUIDELINES FOR THE INTERNS

The objective of this internship is for a visible learning process to take place that could be
quantified into actual performance inputs. These performance inputs then become the basis for
evaluation by Department Heads or their authorized evaluator on the performance of the student-
interns.

The intern is treated like regular employee as far as house rules and regulations are
concerned.

I. ATTENDANCE

 The student-intern is expected to be present on all the scheduled days of internship. If


unavailable, the student intern must notify the HR office immediately.

 The student intern is encouraged to come to work 30 minutes before the scheduled time
of duty to enable to change into the prescribed uniform and report to the work area on
time.

II. DRESS CODE

 The student-intern is expected to be properly attired upon entering the Business


Industry’s premises.

 The student-intern may be assigned to wear corporate attire at work as any regular
employee. This attire should be worn while on duty, together with ID issued by the
company.

CONDUCT AND DISCIPLINE

Every intern must uphold the moral teachings of the school, thus at all times prim and
proper in dealing with co-interns and employees of the business industry.

Every student is expected to be of good moral character. No student intern shall engage in
illicit affair with co-intern and employee. Student, who may have derogatory record from the
Office of the Student Affairs, should seek guidance from the Guidance Counselor and a Clearance
from the Director of Student Affairs, and to the Dean of the College. A written promissory note
should be provided by the student to ensure that meritorious standard is maintained for the school
to protect its name and its community and for the student not to harm the image of the school.

 Observe proper decorum within the business industry premises at all time. There
should be no horse-playing, loud voices or loud laughter’s especially in the locker
room, public hallways and areas.

 Be prepared to be frisked bodily by the security guard and to open bags or package for
immediate inspection.

 Deadly weapons such as gun, knife or any sharp objects should not be found in one’s
possession.

 Follow company policies.

 Be at your assigned post at all times. Do not loiter around the company premises.

 Telephone calls and visitors are not allowed unless reason is of urgent nature.

 The interns must observe the company’s safety rules and regulations. Texting and
calling thru cellular phones, and smoking while on duty or in company premises is not
allowed.

 Avoid pilferage and used of the company’s facilities and equipment without proper
order from the superior.

DUTIES AND RESPONSIBILITIES OF THE PARENTS/GUARDIAN

The Parents/Guardian of the student is the partner of the Internship adviser and the business
industry in supervising and monitoring their activities and progress in their internship.

1. To encourage the student to carry out effectively and efficient the assigned task and
responsibilities.

2. To checked the student interns of their whereabouts and while in the program.

3. To be responsible for the safety of the student interns during their travel from the school to
the internship site and vice versa.

4. To understand and address the financial and psychological needs of the students.

DUTIES AND RESPONSIBILITIES OF THE INTERNSHIP COORDINATOR

The Coordinator should check the performance of every student in their business industry
to ensure discipline and evaluation of the student’s performance.

1. Conduct pre-orientation seminar regarding the requirement of the subject before the
deployment of the student interns to their business industry.

2. Endorsed student to host establishment thru a Letter of Endorsement chosen by student.

3. Check pertinent documents needed for internship.

4. Conduct post conference/evaluation on the performance of the interns.

5. Regularly supervised student interns to monitor their performance.

6. Prepare summary report of the Intern’s performance.

7. Prepare list of establishments/ companies of training.

8. Compile reports to be submitted by the students after the internship.

9. Ensure that all students enrolled in the subject will finish the course on time.

10. Prepare contract/agreement.

11. Schedule date of visit to the business industry.

DUTIES AND RESPONSIBILITIES OF THE BUSINESS INDUSTRY

1. Ensure that the student intern is assigned within the area of scope of the internship program.
Task and duties assigned should be accounting related in majority.

2. Ensure the safety of the student during their internship.

3. Assign immediate supervisor who will monitor the attendance and performance of the
student intern.

4. Inform the school through the internship adviser and or the College Dean of any untoward
incident or circumstances.
