CONTRACT SERVICE
KNOW ALL MEN BY THESE PRESENTS:

This contract of agreement made and executed by and between the Local Government Unit of San
Remigio, represented by HON. MARGARITO C. MISSION JR., Municipal Mayor, and BERNADETTE
ESMERALDA-TOCOYO, of legal age and a resident of Iguirindon, San Remigio, Antique, and a volunteer
teacher at Gov. Evelio B. Javier Memorial National High School.

Whereas the Municipal School Board has available funds to pay for her/his honorarium as a token of
appreciation for services rendered.

WITNESSETH

That the Municipal School Board will pay BERNADETTE ESMERALDA-TOCOYO an honorarium of
PhP6,000.00 per month from August 01, 2019 until December 31, 2019.

In witness hereof we affix our signatures this 25th day of July, 2019 at San Remigio, Antique,
Philippines.
