AFFIDAVIT
I, NESTOR RYAN M. AGUILA, of legal age, Filipino, married and presently residing at 22
Jersey St., Rancho Estate 4, Marikina City, having been duly sworn in accordance with law,
hereby depose and state:
1. That on August 30, 2017, the Security and Exchange Commission (SEC) issued the
Certificate of Partnership for our company, N.R.M. Aguila Glass and Aluminum Supply
Co. with Registration No. PG201729167 and Company TIN 009-799-703;

2. That however, a cursory scrutiny of the name of our partnership, it appears that I
together with my partner inadvertently overlooked the name appeared in the aforesaid
Certificate of Partnership should be “N.R.M. Aguila Glass and Aluminum Supply” and
not with “Co.”;

3. That at the time of the application of the said Partnership Name, my partner and I
wanted the name of “N.R.M. Aguila Glass and Aluminum Supply” only of our
partnership;

4. That we firmly believe that the it is inappropriate to use and include the word “Co.” in
our partnership name precisely because we applied only for partnership and not
corporation;

5. That on this basis, I am executing this affidavit on behalf of my partner to respectfully

request the Honorable Security and Exchange Commission to amend our Partnership
Name from “N.R.M. Aguila Glass and Aluminum Supply Co.” to “N.R.M. Aguila Glass
and Aluminum Supply”;

6. That I am further executing this affidavit to attest to the truthfulness and veracity of the
above mentioned statements.
