Parents Certification of Permission/Waiver

This is to certify that I am allowing my son/daughter to start the practicum for the period
2nd Term A.Y. 2019 - 2020 with The Smile Bar Philippines 2F Uptown Parade, 9th Avenue
corner 36th street, Bonifacio Global City.

It is understood that that he/she will abide by the rules and regulations that may be
imposed by the facilitators/staff-in-charge for the welfare and safety of the group.

I fully agree to waive any responsibility on the part of De La Salle University, the
sponsoring organization/ group and/or the facilitators/staff-in-charge in case of any untoward
incident that may happen to my son/daughter during the duration of the activity.
