QUESTIONS:

1. What are the important nursing assessments for nutrition evaluation and
nutritional requirements during pregnancy?

 Pregnancy influences the nutritional health of the mother and the infant. Nutrition
impacts the probability of both high-and low-birth-weight newborn children and long
dangers of developing metabolic sicknesses in adulthood. A healthy weight during
pregnancy can possibly improve the experience and results of the pregnancy. However,
nutritional assessment and calculation of nutritional requirements pose many questions
including the need for some micronutrients more than macronutrients during pregnancy.
The usefulness of laboratory tests for assessment of nutritional status in pregnant women
is limited by changes in blood levels of nutrients or in nutrient-dependent enzymes or
reactions, which most likely reflect normal physiologic changes that occur in pregnancy
rather than a state of nutrient deficiency.

3. What would be important factors to assess if she is well hydrated?


 Water makes up approximately 60 percent of our bodies and plays an important role in
virtually every bodily function. Water helps with nutrient absorption and delivery, waste
removal, digestion, and body temperature regulation. It even helps cushion your joints. A
pregnant mother needed more water than the average person since water plays an
important role in the development of the fetus. This is because water takes on additional
roles during pregnancy. For example, water is an essential part of the placenta, which
delivers nutrients to your growing baby, and the amniotic sac, which cushions your baby
throughout pregnancy. During pregnancy, inadequate fluid intake over longer periods of
time can also lead to low amniotic fluid, and it may trigger Braxton-Hicks contractions, a
type of contraction that occurs during pregnancy. It can also lead to more serious
complications such as birth defects. A sign of being well-hydrated is having a clear urine
color. If the urine is dark yellow color, it could be a sign of dehydration.

4. What would be the best advice to a known smoker like her, taking into
consideration the teratogenic effects of cigarette smoking?
 The best advice to give to a pregnant smoker is to educate her about the effects of the
cigarette in her baby and for her to understand and realize that smoking is hazard for the
baby’s health. Smoking during pregnancy increases the risk of health problems for
developing babies, including preterm birth, low birth weight, and birth defects of the
mouth and lip. Smoking during and after pregnancy also increases the risk of sudden
infant death syndrome (SIDS). We want her to realize that every action there is always a
consequence.
GORDON’S 11 FUNCTIONAL HEALTH PATTERN

1. HEALTH PERCEPTION-HEALTH MANAGEMENT

 The client said that for her a healthy person is someone who eats a healthy food such as
vegetables and fruits. She drinks water after she vomited. These past few days she kept
on drinking water but not too much. She said taking her kids to the hospital if her child’s
illness will get worsen.

2. NUTRITIONAL-METABOLIC PATTERN

 The client said that she always eats vegetable now than how she eats when she is not
pregnant. She increased her food and fluid intake. Sometimes if they have spare money
she eats meat. She eats 4 times a day including her snack time. Also, she drinks 9-10
glasses of water a day.

3. ELIMINATION PATTERN

 The client takes bowel every morning and afternoon and it hasn’t change even though she
is now pregnant. Every time she woke up she would urinate and urinates at least 2 times a
day. There is no presence of discharge and there is no pain in urinating and defecating.

4. ACTIVITY-EXERCISE PATTERN

 She makes it a habit to walk regularly every morning specially that she is pregnant. When
she is not pregnant she rarely walks because she has to take care of her children. She does
now a light household chore such as sweeping the floor or folding the clothes.

5. SLEEP-REST PATTERN

 She usually sleeps at 9:30 PM and wakes up around 6:00 AM because she has to cook
breakfast for her kids and husband. Now that she is pregnant she always likes to sleep
specially after eating her lunch. She feels like she is deprived of sleep for how many
years.

6. COGNITIVE PERCEPTUAL PATTERN

 The client is a high school graduate. Client is attentive and oriented about the place, time
and the situation. She has no difficulty in hearing and does not use any hearing aid. Her
vision is normal; she does not wear any eyeglasses. She perfectly understands and
remembers the things I said.

7. SELF-PERCEPTION-SELF-CONCEPT PATTERN

 Client said that she is happy with herself. She said that there is a physical change now
that she is pregnant. She grew bigger than her usual weight. Sometimes she felt ugly but
her husband always reminds her that she is beautiful in every way.

8. ROLE-RELATIONSHIP PATTERN

 They have their own house. Her mother sometimes visits her. They are married and have
4 children. She and her family have a tight relationship and they always supports her
specially when problem arises.
9. SEXUALITY-REPRODUCTIVE PATTERN

 The client does not use contraceptives and they are sexually active.

10. COPING-STRESS TOLERANCE PATTERN

 She said that she feels stress when she has a lot of things to do at home. To relieve her
stress she would listens to music or watches television. Sometimes she would talk to her
husband or to her mother but she always talks to her mom about her problems because
she thinks that her mother knows best.

11. VALUE-BELIEF PATTERN


 The client is Muslim. And she prays five times a day. So, she believes that the most
important or valuable in her life right now is her children and her family. They make her
happy in every possible way.
