3. Occupancy Permit
4. Contract of Lease (if Lessee)
5. Barangay Clearance

Pay the assessed fees and charges in Window 1 5
or Window 3 CATHERINE IBARRAT
(The applicant may choose to pay on an annual, semi-annual minutes
or quarterly basis)
Submit the duly signed application form and
Official Receipt for the issuance of Business 5 JESSIE ASPIRAS
