AFFIDAVIT OF EXPLANATION

I, JONILA L. LLAMADO, Filipino, of legal age, with residence at 43 G. Fernando St.

Malanday, Marikina City, after being duly sworn to in accordance with law, depose and
say:

1. I was born with the complete name Jonila Lazaro Llamado, on September 15, 1998
as stated in the birth certificate attached to this affidavit as “Annex A”;

2. My parents are Lanie Lazaro Llamado and Joel Masiglat Bautista, as stated in the
aforementioned birth certificate;

3. Since my parents are not legally married, and since I was born prior to the
effectivity of Republic Act No. 9255, I am not allowed to make use of my father’s
surname;

4. Thus, as an illegitimate child, I have been using my mother’s surname in all my

identifying documents;

5. Further, I am executing this affidavit to establish that I have only one sister, Jonina
Lazaro Llamado, whose birth certificate is attached to this Affidavit as “Annex B”;

6. As such, I am executing this affidavit of explanation to establish that despite the

similarity in our surnames, my mother Lanie Lazaro Llamado is indeed my mother
and not my sister;

7. That I am executing this affidavit to attest to the truth and veracity of the foregoing
facts and for whatever legal purpose it may serve.
