TERMS AND CONDITIONS:

• BORROWER agrees that the devices shall be used only for purposes mentioned above and shall be
operated in accordance with the training instructions provided by PACKETWORX. BORROWER shall
not modify or alter the devices in any manner. Furthermore, BORROWER shall not use the devices
for any purpose or activity that violates law, public policy, morals, and good customs.

• The connectivity, internet, electricity, and other necessary costs required to run the devices shall
be borne by BORROWER.

• BORROWER shall be personally liable for the lost and damage of the equipment/devices.
BORROWER shall pay all repairs and replacement costs, as indicated in the form, from damage or
loss of the devices.
