AFFIDAVIT OF LOSS

I, WINDEL V. ENCAJONADO, of legal age, Filipino, married to MARINA L.

ENCAJONADO and resident of Brgy. Danao, Janiuay, Iloilo, represented by his Attorney-in-fact,
MARINA L. ENCAJONADO, after having been sworn to in accordance with law, hereby depose
and state:

1. That I am the purchaser of the delinquent real property declared under the name of
ANTONIO MALLO known as Lot No. 5493 Janiuay Cad. covered by Original
Certificate of Title No. 60860 containing an area of TWENTY-SEVEN THOUSAND
FOUR HUNDRED TWENTY-THREE square meters (27,423 sq. m) and located at Danao,
Janiuay, Iloilo.

2. That a Final Deed to Purchaser has been issued to me by the Office of the Provincial
Treasurer, Province of Iloilo, dated November 22, 2020, and attached hereto as
Annex “A” is the Final Deed to Purchaser and made an integral part of this Affidavit;

3. That the original transfer certificate of title was not turned-over to me and that despite
exerting earnest and diligent efforts to locate said title, the same is believed to lost
and could no longer be found and believed to be beyond recovery;

4. I attest to the truth of the foregoing statements to the best of my own personal
knowledge and based on authentic documents or records. I am executing this Affidavit
of Loss to support my application for a new owner’s duplicate copy of Original
Certificate of Title No. 60860 as well as for all lawful intents and purposes it may serve;
