AFFIDAVIT OF CAPITAL INVESTMENT

That I, JENNY ROSE M. RAMOS, of legal age, Filipino, married, resident of and
with postal address at Nilasin 2nd, Pura, Tarlac, Philippines, after having been duly
sworn in accordance with law, do hereby depose and say;
1. That I am the owner/proprietor of MENMOS CANTEEN located at Poblacion
2, Public Market, Pura, Tarlac, with Certificate of Business Registration No.
03634543; photocopy of said Certificate of Registration is hereto attached for
reference;
2. That in the said business I have a capital investment of P100,000.00;
3. That I am executing this affidavit to attest to the truth of the foregoing
statement and to inform the concerned agencies that the said business of
MEN – MOS CANTEEN located at Poblacion 2, Public Market, Pura, Tarlac
has a capital investment of P100,000.00.
