Greetings in the name of Safety and Health!

We are happy for your interest in CPS5 Corporation in rendering services for the conduct of
Occupational Safety and Health Training as stated on RA 11058, “An Act Strengthening
Compliance with Occupational Safety and Health Standards prescribed by DOLE.

Objectives: By the end of the end of the ten (10) hour session, the participants shall be able
to:

o To fully understand the importance of safety not only in the workplace but also in the
participant’s personal lives;
o To understand what professional management is all about and to know how it can
help in the performance one’s job more efficiently;
o To be able to quantify safety/accident related activities through common standards;
o To provide supervisors with a systematic tool in conducting inspection and
evaluation;
o To develop ways in improving work output through motivated behaviour.

Training Schedule and Venue:

 The training is ten (10) hours on Occupational Safety & Health Training for SO1 Safety
Officer.
 The date will be coordinated with us and the venue will be held at your office.

Cost of Training Fee/Payments:

 Cost is Php 2,500.00 per participants inclusive of 12% vat.

 Certificates of Completion as Safety Officer1 (SO1) and Train the Trainer will be
issued to individual participant.
 Cash or Check Payments should be payable to CPS5 Corporation (TIN 008-594-783-
000 VAT Reg.) at the end of the training.
