As a grantee, we would like to formalize the terms and conditions covering your tuition/token fee exemption grant as
follows:

1. Your grant shall consist of (FULL/PARTIAL) TUITION/TOKEN FEE EXEMPTION.


2. This grant is subject to semestral review of your academic performance/achievement.
3. For retention, you must meet the following conditions every semester:

a. Maintain a semestral grade


ACADEMIC
COAHS Academic Scholarship grantees' GWA must be 1.75 , with no grades lower than 2.0
(in all Tier)
Dean's Lister Scholarship grantees' GWA must be 1.00-1.75 with no grades lower than 2.0

SPECIAL
Sister City Scholars grantees' GWA must be 2.00 or higher with no failing grades
MINES grantees' GWA must be 2.50 or better with no failing grades and must render
voluntary service or participate in at least five (5) Scholars' Society activities

Has rendered significant contribution to the University/City of Makati
b. Must not drop any subject, nor cancel enrollment at the time he/she enjoys the grant;
c. Must carry full load as prescribed by the gratee's program of study
4. Your grant shall automatically be forfeited in case of expulsion from the school or if you discontinue your studies.
5. You must abide by the program-related requirements aside from those stipulated above, such as, but not limited
to, attendance in fellowship activities and other out-of-school activities that may be organized for grantees,
reporting of grades, monitoring and evaluation of academic performance, and membership to Scholars' Society.

Thank you for applying for UMak's Scholarship and Tuition/Token Fee Exemption Program!

Kindly print the attached filled-out Tuition/Token Fee Exemption Application Form in an A4 size bond paper and submit the same
together with the other requirements to the Testing, Admission, & Scholarship Center (TASC). Office hours are Mondays-Fridays,
8:00 AM-5:00 PM. Please see below for the list of requirements per type of exemption:

Basic Requirements (two copies each):

1. Printout of the duly accomplished Tuition/Token Fee Exemption Application Form


2. Residency Verification Form (c/o Accounting Office) for the following:
Incoming Second Year students
COAHS students
MTPLC
3. 2 x 2 I.D. Picture (for NEW Scholar Applicants)
4. Encoded Advising Slip (for MTPLC applicants)

Additional Requirements (two copies each):

Dean's Lister - Partial a. Photocopy of the Report of Grades from the


previous semester UMak's College
b. GWA of at least 1.75, no grade lower than 2.00, and
a grade not lower than 2.50 in P.E. and N.S.T.P.
c. Endorsement from College Dean

Once you have submitted your complete documents, it will be endorsed to the Office of the Vice President for Academic Affairs for
approval which usually takes one (1) to three (3) working days. Once we have the result of your application, we will inform you
through email. Likewise, you are advised to return to the TASC Office on the date indicated on your claim stub for updates
regarding the status of your application.
