JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

RE: ONE AND THE SAME PERSON

We, MARY and DAYANARA, both of legal ages, Filipinos, and residents of
Barangay Purok 3 Barangay -------------- after having been sworn in accordance with
the law hereby depose and state that;

1. We personally know HYDI who was born on January 2, 1981 because she
is our longtime friend and acquaintance at Purok 3 Barangay ---------, and
the fact of her birth is registered at the Local Civil Registrar of ------.

2. We know for a fact that the person by the name of HYDI T. -who was born on
January 2, 1981 as appearing on her Certificate of Facts of Birth and the
person by the name of HAIDEE who was born on January 1, 1981 as
appearing on her OWWA Claim Form and Certificate of Marriage refer to one
and the same person and that her true and correct name is HYDI T. -whose
date of birth is January 2, 1981;

3. We are not in any way related to any of the foregoing person within the fourth
civil degree either by consanguinity or affinity, nor do we have any pecuniary
interest in this matter whatsoever;

4. To remove confusion as to the name, we hereby declare that the name

"HAIDEE” who was born on January 2, 1981 and HYDI born on January
1, 1981 refers to one the same person;

5. We are executing this affidavit to attest to the truthfulness of the foregoing

statements and to establish the real name of HYDI T. and for all legal intents
and purposes it may serve.
