AFFIDAVIT OF GUARDIANSHIP

I, ________________________, Filipino, of legal age, single, and a

resident of Barangay _______________________ after having been duly sworn to
in accordance with law, hereby depose and state:

1. That I am the elder sister of ______________________inor, who was born

on April 6, 2003;

2. Our natural and/or biological parents are _______________ who is

presently in Manila while our mother is ______________ who is an Overseas
Filipino Worker (OFW) in Hongkong;

3. Consequently, when our parents went for work, my minor sister was
left under my care and custody;

4. As such, I became the legal guardian of my sister considering that our

parents are far from us and no other person will take care of her;

5. I am executing this Affidavit to apprise all those concerned of the

foregoing facts and for whatever other legal purpose it may serve.
