Jessica Esteves, presently residing at Matahimik St.,
Quezon City;

3. I hereby recognized and acknowledge therefore the

said Joanna dela Cruz as my daughter and to have an
obligation to support her needs and other matters
pertaining to her welfare;

4. This affidavit is executed to attest the truth of the
