AFFIDAVIT

I, AURELIO M. MACALINAO, JR., of legal age, Filipino, married

and a resident of #4 Redeemer St., New Era, Quezon City after having
duly sworn on oath in accordance with law hereby voluntarily depose
and say that:

1. I am the father of JANN ANDREII C. CRUZ, a minor, who was

born on March 27, 2002 and at present he is in my custody and support;

2. The mother of the said minor, Ma. Angelica Centeno had

abandoned him sometime in 2005 when he was only two (2) years old;

3. My said wife is nowhere to be found and we have no knowledge

as to her circumstances and belongings;

4. I am executing this Affidavit for purposes of Passport

Application and for whatever legal purpose this may serve best.

IN WITNESS WHEREOF, I have hereunto set my hand this 4 th day
