AFFIDAVIT

We, BEETHOVEN M. LUCEÑO and LORELIE T. LUCEÑO, both of legal ages, Filipino
citizens and residents of Tumaga, Zamboanga City, Philippines, after having been duly sworn to
in accordance with law hereby depose and say that;

1. Sometime in the year 1980, the affiants met and after a certain period of courtship, were
married on 17 January 1981 at 8:30 o’clock in the morning at St. Anthony Parish, U.P,
Sta. Clara, Lamitan, Basilan;

2. Their marriage was duly solemnized by Rev. Fr. Alfredo E. Oyao who was the Assistant
Parish Priest of the above-mentioned church at that time;

3. The celebration of the marriage and the ratification of the same is evidenced by the
Marriage of Contract duly signed by the affiants, the solemnizing officer and their
witnesses, a machine copy thereof is hereto attached as Annex “A”;

4. Some time last month, LORELIE T. LUCEÑO went to the National Statistics Office
(NSO), Zamboanga City, to obtain a copy of the Marriage Contract but no such entry
was found in their records;

5. LORELIE T. LUCEÑO, also went to the Office of the Civil Registrar of Isabela and
Lamitan, Basilan but no entry was also found in their records;

6. By virtue of the above foregoing, there is a need for the affiants to file for the late
registration of the fact of their marriage before the Civil Registrar of Lamitan, Basilan so
that the same may be forwarded to (NSO), Zamboanga City to update their records;

7. We make these statements under oath and in good faith in order to establish the fact
that there was no records of the fact of our marriage found in the Office of the Civil
Registrar of Isabela and Lamitan Basilan as well as NSO;

8. We execute this Affidavit to attest to the truth of the foregoing facts based on personal
knowledge and authentic records in compliance with the requirements for the late
registration of our Marriage in the Civil Registrar of Isabela, Basilan and for whatever
legal purpose it may serve.

IN WITNESS WHEREOF, we have hereunto set our hand on this 25 th day of June 2009 in
the City of Zamboanga, Philippines.
