AFFIDAVIT
OF CORRECTION

I, MA. LYN OLARTE BEZARTE, of legal age, single, and residing at B17
L34, 14th St., the Legian 2, Ph. A, Carsadang Bago, Imus City, Cavite, after having been
duly sworn to in accordance with the law, do hereby depose and say:

That I am the registered owner of Transfer Certificate of Title TCT No. 057-
2014010897 situated at the above mentioned address;

That however, there was a found deficiency on my afore-said title, whereas, on

the annotation for Encumbrance with entry No. 2016059854 the book No. indicated
therein was erroneously written as CDLXII when I found out that it should be CDXLII;

That the discrepancy on the Book no. annotated in my Transfer Certificate of Title
might be due to the inadvertence of the person who typed the entry hence can be rectify
accordingly;

That I hereby request the Registry of Deed for the Province of Cavite to correct
the said entry from Book No. CDLXII to CDXLII;

That I am executing this affidavit to attest to the foregoing facts and to prove the
fact of the found deficiency in the Transfer Certificate of Title which I am the registered
owner and ask the Registry of Deeds of the Province of Cavite to correct the annotation
for Encumbrance with entry No. 2016059854 from Book No. CDLXII to CDXLII and for
whatever legal purpose this may serve.
