AFFIDAVIT OF SERVICE

I, JAY LUGAY, Filipino, of legal age, married, and resident of Brgy. De

Ocampo, Trece Martires City, Cavite, after being duly sworn to in accordance with law,
depose and state: That - -

1. That I hired the services of Hermosa Law Office with Office address at G/F
K-Ann Bldg., Cong. Ave Burol 11, Dasmarias, Cavite for writing a demand
letter upon the person of NERISSA OLFINDO with address at Blk 68 Lot 1
Phase 6, Fatima 3, Dasmarias City, Cavite.

2. That the cause of my demand is for sum of money against NERISSA

OLFINDO.

3. That I personally served the copy of the demand letter at the residence of
NERISSA OLFINDO but the she refused to sign the same.

4. That I am executing this Affidavit to attest to the truth and veracity of the
foregoing and for whatever legal purpose the same may serve;
