AFFIDAVIT OF LEGAL BENEFICIARY

I, ANAMARIE A. QUIÑANOLA, of legal age, Filipino citizen, single and a

resident of Barangay Rizal, Dimasalang, Masbate, after having been duly sworn
to in accordance with law, hereby depose and say, that:

1. That I am applying as trainee in the Philippine National Police.

2. That as one of my requirements, I am declaring my parents,

Ludegario A. Quiñanola and Villa A. Quiñanola as my legal beneficiaries, herein
attached my Certificate of Live Birth as ANNEX “A”.

That I am executing this Affidavit to attest to the truth of the foregoing and
for whatever lawful purpose that it may serve.

Further affiant sayeth none.
