Information Technology College, hereby permit/authorize him to undertake On-the-Job

Training (OJT) as an academic requirement of graduation.

I agree that the said training be conducted off campus this 2 nd semester, School

Year 2018-2019 in any industry, government or non-government institution, where

Electronics Engineering and any related work is practiced as a profession. I understand

that the conduct of the training will be guided by the rules, regulations, policies and

standard operating procedures of the College and the Cooperating Agencies.
