Please be reminded of your lacking CO. Those who are not done yet with their
4 CO must provide their final schedule. I together with our HT will be out this
Wednesday, February 26, 2020 for the final submission of the SIP/AIP and Clinical
Supervision, respectively. In line with this those who had their schedules on the said
date must give another preferred date for the observation.

SALN and PDS are required to be sent to the Division Office in soft copy (PDF
file form). In line with this since we were not able to pass our SALN yet you are
required to submit again in 4 copies of your SALN and a complete set of your PDS
duly signed by our Administrative Officer for scanning. This is due on Thursday,
February 27, 2020 (school based). Then due for submission on Friday, February 28,
2020 (division level).

Another reminder is your CS FORM 86 (Health Examination Record) which

was already due last February 20, 2020. Please submit it in the office on Thursday,
February 27, 2020 for consolidation and submission at the Division Office on
February 28, 2020.
