Mentor: Dr.Vishnu Kalidindi

Date Description
1/02/20 I initiated my endeavors in the mentor approval
process by contacting Ms.Pinal Patel, an x-ray
technician at Dr.Kalidindi’s clinic, and
accumulated information pertaining to how to
complete Baylor Scott and White’s preliminary
application. Throughout the process, Ms.Pinal
Patel was extremely helpful in directing me to
different resources.

1/04/20 I ran into my first obstacle in the application

process due to issues with a sharepoint site. Once
again I was able to reach out to Ms.Patel to better
understand how to approach a solution and was
redirected to the office manager, Ms.Bartlett, who
provided with a follow up application form.

1/15/20 I conducted a follow up phone call with

Ms.Bartlett and she was extremely kind in helping
me understand how the process had changed due
to a recent merger. Furthermore, Ms.Bartlett and I
were able to re-input the information into the
database in order to help expedite the process.

1/30/20 I was able to reach Ms.Bartlett again and learned

that because of the need for a social security
number, the process may be getting delayed.
Therefore, I was able to fill out a new application
form provided by the onboarding team with the
necessary SSN information as well as update
Mr.Pirtle on my current situation.

2/05/20 I composed a hand-written thank you note to

Ms.Bartlett to express my sincere gratitude
towards her unparalleled assistance as well as left
a voicemail to ensure that the information was
received.

2/11/20 I conducted another courtesy call to Ms.Bartlett to

communicate if there was anything that needed to
be done by myself or Mr.Pirtle to help expedite
the process and was happy to learn that the
 application was being processed.

2/27/20 Ms.Bartlett helped facilitate an initial mentor

meeting with Dr.Kalidindi and I had a fantastic
initial opportunity to review the parameters of my
original work, mentorship expectations, as well as
get to meet Dr.Kalidindi and Ms.Bartlett.
