GENTLEMEN:

Due to my retirement last October 28,2012, I would like to withdraw my regular membership
from the cooperative and consequently my Capital Build Up.

I therefore authorize BACGEM to deduct from my Capital Build Up/shares the amount due to
pay for my educational loan with the cooperative.

Thank you for the chance of enjoying the services, camaraderie and all the learning experiences
gained from beIng a member of BACGEM.

I hope to be a member again even on an associate category.
