The discrepancies in the On Line CMO Reg.

/Ren Form format

In the beginning, after having chosen the option- “Renew Registration Application Form”, accepted the terms and
conditions, it asks- have you generated your old registration certificate from this portal ? One selects- No ... it again
makes you affirm that you are sure. Then, the main ‘Application Form’ opens with different fields as below-

1- DETAILS OF PREVIOUS CERTIFICATE

Should be - Eg.
Old Registration Number - Original CMO Registraion number UP/AGR/2004/AL/7**
Old Certificate Number - Last years Renewal Certificate number UP/AGR/2004/AL/7**/19-20
UPLOAD old Certificate -Upload Last year Renewal Certificate only

2- DETAILS OF MEDICAL ESTABLISHMENT

Place of Establishment *Own *Rental *Other specify eg.-Parental
Type of Land *Commercial *Residential *Mixed Population
Category *Drop box options should add “Consultation Chamber”
Upload Building Structural Layout *Remove this option

3-ADDRESS OF MEDICAL ESTABLISHMENT

ADDRESS PROOF UPLOAD *Electricity Bill/Telephone Bill/Rent Agreement/Registry/aadhar/PAN/DL/Affidavit
or other standard IDs
4- DETAILS OF MEDICAL SERVICES
*use in case of multiple services *remove this condition
*Only a blank box is enough to enumerate the facilities & Services

5-DETAILS OF PERSON IN CHARGE
6-DETAILS OF DOCTOR
7-DETAILS OF PARAMEDICAL STAFF
8-TYPE OF FACILITIES OFFERED
*no need to ask- Institution Name & Name of Central/State Council
*If samecas 5, then ONLY one field (out of 5 /6) is enough another should disappear
*Registration type column is redundant, It will appear in Registration number
(If registered with UPMCI & MCI both, then how would one select??)
*This field should be OPTIONAL as Clinics / Consultation Chambers don’t require it
*Remove the Mendatory ->* mark
*Required for Nursing Home/Hosp/Diagnostic centers
*No need, as already asked in MEDICAL SERVICES field

Have you obtained NOC frm Pollution *No need for Consultation Chamber/Clinic without BMW excretion
