We, the second year nursing students of Notre Dame of Dadiangas University have our Delivery
Room Exposure in Dr. Jeorge P. Royeca Hospital last January 14, 15, 21 and 22 of 2020. We are
obliged to comply our requirements including chart information of our patients.
In relation to this, we would like to borrow our patients’ charts and medical information for us to
acquire necessary information to complete our requirements. Rest assured that we had our patients
signed the consent form in regards with the data privacy act and to observe the patient’s privacy and
