PAY PREMIUMS WITH PRE-TAX DOLLARS


The Container Store participates in an IRS Section 125 plan that allows employees to pay for certain expenses with pre-
tax dollars. Our plan allows you to pay for your medical, dental and vision, accident and In-Hospital Care insurance
deductions with pre-tax dollars through payroll deductions. You cannot cancel your coverage during the year unless
you experience a Qualifying Event, or during the annual TCS Open Enrollment or the Affordable Care Act (ACA) Open
Enrollment. Information regarding Qualifying Events will be explained later in this guide.

The Summary Plan Documents (SPD) provide detailed information concerning all of our part-time Medical, Dental, Vision,

their families. In the event of a discrepancy between the information presented in the Benefits Guide and the Summary Plan
Document, the official Summary Plan Document will prevail. The Container Store reserves the right to change, amend, or end
any benefits at any time, for any reason.

ELIGIBILITY

Waiting Period
You have 31 days from your date of hire to enroll for benefits. Your coverage will become effective 1st of the month following
30 days of employment.

QUALIFYING EVENTS FOR CHANGES TO YOUR BENEFITS


Qualifying Events allow you to make changes to your elections before or after Open Enrollment
as long as you submit the changes within 31 days of the Qualifying Event.

MEDICAL
The Container Store offers part-time employees with minimum essential medical benefits that are ACA compliant.
This Summary of Benefits only represents an overview of your medical benefits, and is subject to change. Complete

WELLNESS PLUS PLAN - GROUP & PENSION ADMINISTRATORS


This plan is focused on preventive wellness coverage, minimal medical provider visits, and a generic prescription program,
with a brand name prescription discount card. The Wellness Plus Plan is part of the PHCS Network.
The Wellness Plus Plan is intended to cover all Affordable Care Act (ACA) compliant wellness and prevention services that
are considered Minimum Essential Coverage. The Plan does NOT include Hospitalization, Emergency Room, or Mental
Health services. Nor does the plan cover mental health medications, or many other name brand medications. Some
generics are offered at a $0 copay, and other medications are offered at a discount only.

All mental health / substance abuse treatment and services (including but not limited to diagnosis,
testing and any related services) and mental health/substance abuse prescription drugs are excluded
from this Plan.
State of Massachusetts employees: The Wellness Plus Plan does not meet the required MCC
Standard of Massachusetts. If plan is elected, you must indicate “No MCC/None” on line 3
of the Schedule HC. If you had access to affordable insurance that met MCC, but did not
purchase you may be subject to penalty. For more information please visit Mass.gov.

EXAMPLE
Christina is planning for her annual Wellness and Prevention care visit - all is covered for allowable services including
Mammograms for participants over the age of 40. However, the physician states that she needs to have a additional lab work
outside of the standard wellness and prevention screening and an MRI for some joint pain she has been encountering. The
additional lab work will not be covered under the plan as wellness and prevention and will be the individual’s responsibility
as it is outside of the parameters of standard wellness care. The MRI is also not covered as this is not considered a service
under the wellness and prevention coverage. The individual can contact Health Advocate (866-272-6009), an employer-
paid service, to assist with locating a low cost imaging facility in her area to understand the out-of-pocket expense that
is required.

DENTAL
The Container Store offers two dental plans that are administered by Aetna. The DMO Dental and PPO Dental Plans.
Please visit the Aetna website to locate participating providers at www.aetna.com.

DMO PLAN (Dental Maintenance Organization)


It is required to select a primary dentist, and it is necessary to receive referrals for specialty care. The Aetna DMO plan does
not have a plan deductible or an annual maximum limit for coverage. The DMO plan has copays on type of service. You may
also locate your information by visiting aetna.com.

PPO PLAN (Preferred Provider Organization)


It is not required to select a primary dentist and referrals are not necessary to receive specialty care on the self-insured
PPO plan. Aetna will issue your member ID number via regular mail in the form of a letter as Aetna does not issue
dental cards.

The Container Store offers vision coverage through EyeMed. The vision plan offers access to a large network of participating
eye care providers and provides discounts on lenses, frames and conventional contact lenses.
The EyeMed Plan offers in-network benefits, as well as out-of-network reimbursements. EyeMed includes a dynamic network
that includes private practitioners and the nation’s leading optical retailers; LensCrafters®, Sears Optical®, and JCPenney

The Container Store offers Voluntary Life Insurance to all part-time employees. The waiting period is the same as health
insurance: You have 31 days from your date of hire to enroll for benefits. Your coverage will become effective the 1st of
the month following 30 days of employment.
Premiums are paid by the employee through payroll deductions on an after-tax basis.

BENEFIT REDUCTION DUE TO AGE


Life insurance benefits reduce the original elected amount by 35% at age 70, by 55% at age 75 and by 70% at age 80.
All coverage terminates at retirement.

ACCIDENT
The Container Store offers Accident coverage to part-time employees through Aflac. An accident insurance plan provides
benefits to help cover the costs associated with unexpected bills. Accident insurance is a way to stay ahead of the
medical and out-of-pocket expenses that add up so quickly after an accidental injury – not just for emergency treatment,
hospital stays and medical exams, but for other expense you may face, such as deductibles, co-pays, transportation
and lodging. When an enrolled employee has a covered accident expense, Aflac will send cash benefits directly to
the employee who decides the best way to spend them. It’s as simple as that. This plan includes accidental death and
dismemberment. Please note if you have an on-the-job injury you must notify your manager immediately or contact
riskmanagement@containerstore.com. All non-work related AFLAC claims must be submitted directly to www.AFLAC.com.

IN-HOSPITAL CASH PLAN OPTIONS
The In-Hospital Cash Plans are a great way to help with out-of-pocket expenses when you have an in-patient hospital stay.
This optional coverage will pay a lump sum to the employee, or provider, when there is an in-patient hospital stay for a
minimum of 24 hours. If the patient remains an in- patient longer than 24 hours, the plan pays a daily amount, based on
which plan is selected, for a limited number of days/occurrences per calendar year. Note: “in-patient” status is defined as
having Room and Board charges for each 24 hours of the hospital confinement.

IN-HOSPITAL CASH PLAN BENEFIT


Benefits are paid on the first day of a covered stay. ICU, substance abuse, mental health and nursing facility stays are
included with the hospital stay benefit. The ICU benefit pays double the amount of the hospital amount. Each facility has
a calendar year maximum number of days as selected, with 500 days per lifetime unless otherwise noted in the policy.

IN-PATIENT HOSPITAL ADMISSION BENEFIT


This benefit pays for admission to a healthcare facility for a minimum of 24 hours when confinement is medically necessary
and is the result of a non-occupational illness or injury. All 3 plans offer a $1,000 benefit for hospital admission. The hospital
admission benefit for Plan 1 is limited to one hospital admission per year and Plans 2 and 3 are limited to three hospital
admission benefits payable per year regardless of any other in-patient hospital benefits available to the insured. However,
the daily benefit is still available for up to 90 days per year.

SURVIVOR BENEFIT
If an employee dies while insured, any covered dependents will be extended benefits without premium payments for up
to two years after the employee’s death. This is as long as the plan remains in force and the covered dependent meets the
coverage requirements in the policy.

PHARMACY DISCOUNT PROGRAM


If an employee does not participate in the part-time medical and pharmacy plan Symetra offers a discount from usual and
customary drug charges to the eligible person when prescriptions are purchased through a contracting pharmacy. Please

401(k) RETIREMENT PLAN
We value our employees and the contributions they make to the company. The Container Store offers a 401(k) plan to
assist employees with their retirement savings. You are eligible to participate in the plan when you complete 11 months of
active service, rounded to the 1st day of the next quarter. If you have a qualified 401(k) plan from your previous place of
employment, you may roll those funds over to The Container Store plan. You can access the 401(k) Rollover Form on the
TCS Intranet.
When you are eligible to participate, you may contribute 1% to 80% of your gross pay to the plan through pre-tax payroll
deductions, up to the maximum amount determined by the Internal Revenue Service. You may review the Summary
Plan Description and the Qualified Default Investment Alternative Notice on The Container Store Payroll website at
https://mypayroll.ultipro.com.
If you reach age 50 or older during the calendar year, and are making the maximum plan or IRS pre-tax contribution,
you may make additional “catch up” contributions each pay period.
