 Minutes of PTA Meeting(s) with Proof of Attendance
 Report Cards signed by parent’s/guardian’s in the First Quarter
 At least 3 samples of corrected test papers of the same 3 learners in the
same learning area with parent’s/guardian’s signature and date of receipt (may
be provided from any of the other quarters)
 Communication letter sent to parents or guardians (may be provided from any
of the other quarers)
 Plus Factor Certificate (may be provided from any of the other quarters)
 Any 1 of the following:
– Class Records/Grading Sheets
– Summative Assessment Tool with TOS, frequency of errors and identified least
mastered skills
– DLLs showing index of mastery
– Computation of a learner’s written works with summary of results and
signature of parents

2nd Quarter (August to October 2019)

 Classroom Observation Tool 2 (COT Indicators 1-5, 7-8)
