2. Procedure of how to complete the test
a. The teacher will grab 2-3 students at a time, and ask them to perform the
task at the other end of the gym; the teacher should observe one student
at a time
b. Make sure to demonstrate the task and state what you are looking for
c. Provide a practice trial
d. The student should perform three trials
e. Record if the student performs the component on the record sheet
i. Throwing
1. Mark off a line 25 feet away from wall
2. Tell students to throw tennis ball hard at the wall
3. Look for performance criteria
ii. Kicking
1. Mark off a line 30 feet and 20 feet away from wall
2. Place balls on the 20 foot line and tell students to stand on
other
3. Tell students to kick the ball hard toward the wall

3. How YOU will complete the test in the classroom (eg. stations, all at one time)
a. Students will be asked to pass back and forth with a partner on one side of
the gym, either kicking or throwing depending on the movement being
tested on.
b. The teacher will grab 2-3 students at a time, and ask them to perform the
task at the other end of the gym
c. Encourage the students to give their maximum effort
d. The teacher should observe one student at a time
e. Have a rubric with each kids name on it and the basics of the skill that they
are being tested on, so you can quickly check mark if they are performing
that skill
4. Dates you plan to assess- Pre and posttest (needs to be in the first 3 weeks of the
semester, posttest should occur in one of the last three weeks of the semester)
a. On Monday of the third week for pre test
b. On Monday of the fourteenth week for post test

6. Assessment Report Template.


During this semester, your student completed four of the test of gross motor development/skills.
Two tests were assessed by me, Molly Willis, and the other two by Ms. Daubert; We chose to
test your student in two locomotor skills and two object control skills. This test is designed to
assess and monitor your child’s gross motor skills; it is an easy way to assess as it doesn’t take
much time and you simply check if you see the performance criteria being met. The purpose of
these tests are to assist us in identifying any student who may be behind their peers in gross
motor skill development. We are able to use the information of this assessment to develop
instructional programs, monitor progress, and evaluate instruction. Below we have attached
your students’ scores and progress.

As you can see, your student has improved in most of the criteria of these performances and
appears to be learning and developing these skills as they school year goes on.
