CASE 1

1. What do you think is BV’s diagnosis? Defend your answer (5 points)

- Since the patient has severely swollen tonsils, a test called Throat Swab should be done to get a sample of
secretions on the child’s throat to check for streptococcal bacteria. The doctor rubs a sterile swab over the back of
your child’s throat to determine if there is a bacterium that causes the infection

- An order for a CBC test is done in order to identify whether there is an elevation or decrease in the
patient’s blood as this will indicate whether an infection is more likely caused by bacterial or viral agent.

3. Give the rationales for the doctor’s order. (10 points)

- Every order of the doctor has specific reason geared towards the problem of the patient

- More importantly the nurses won’t do anything without the doctor's order for the sole reason that they
will not be held liable for whatever complication it may cause to the patient, except the doctor alone

- The doctor's orders are the keys to the full recovery of the patient

- Every order of the doctor should be in black and white or should be documented for legal purposes

- No other order should be carried out by the nurses except that from the doctors

4. What is the reason why the surgery recommended cannot be done immediately upon consult even if she is
already suffering much from the condition? Why should it be scheduled later on? (5 points)

- VS bordering should be on maximum normal. There is a need to complete the medications given while
observing if there will be changes for one week. Also, referral to a throat specialist is advised.

- Patients apprehension regarding the upcoming quiz bee

4. What could be the possible cause of his frequent respiratory infection? (5 points)

- Since the mother did not seek consultation and admitted that she has had only 2 pre-natal check-ups over the
course of her pregnancy, the child probably was lacking significant nutritional needs of the body. The mother
possibly didn’t choose the right food during the course of her pregnancy. Also RR was born by a mother at 35 years
of age which could pose certain complications for the baby.

CASE 3

3. What would be the cause of his knee pains? What is this condition and why does this happen? (10 points)

- The condition of DC is called Hemophilia A which is an X-linked, recessive disorder caused by deficiency of
functional plasma clotting factor VIII (FVIII), which may be inherited or arise from spontaneous mutation. It is an
inherited bleeding disorder in which the blood does not clot properly. This can lead to spontaneous bleeding as
well as bleeding following injuries or surgery.

- Hemophilia is caused by a mutation or change, in one of the genes, that provides instructions for making the
clotting factor proteins needed to form a blood clot. This change or mutation can prevent the clotting protein from
working properly or to be missing altogether.

- DC experiences knee pains because of bleeding into the joints which causes swelling and pain or tightness in the
joints; that includes the knees, elbows, ankles.
