(BPMW) basis the business licenses/supporting documents provided by me
and the same are correct and authentic.
4. I shall purchase the goods from BPMW for trade/business/institutional
consumption and not for personal consumption.
5. I undertake that any person authorized by me to hold the Primary Card
or the Add-On Card shall also buy only for the purposes specified above, and
not for personal consumption and that i shall be responsible for any act or
omission by such card holders.
6. That I have thoroughly read and understood the Terms & Conditions of
BPMW Membership Application Form. I further undertake to abide with the
said Terms & Conditions and any further instructions issued by the BPMW
from time to time.
