Dear Sir/Madam;

This is to inform you that your contract at (insert name of barangay and municipality) with

started five (5) days ago but no delivery has been made to this date. In lieu to this, we would like to
remind you that the deadline of delivery of the materials listed below is on (insert date of deadline
of delivery stated in the contract). Failure to deliver the materials prior the deadline shall be
subjected to liquidated damages which is equivalent to the amount of 1% of every day delay
(weekends and holidays included) of delivery.
