AFFIDAVIT OF WITNESS

I, MARIA DE CASTRO, 39 years of age, single, resident of Leonarda, Tugeugarao City,

after having sworn in accordance with law, do hereby depose and state THAT:

1. I am a Medical Doctor by profession and registered with the Bureau of Internal Revenue,
ROD 2, Tuguegarao City with Tax Identification Number 1234-5678-00;
2. I am currently working as Medico Legal Officer at Tuguegarao People’s General Hospital.
3. On June 21, 2019 at 9 o’clock in the evening, I examined a person named Joshua Catuiran,
male, single and a resident of Luna Street, Ugac Sur Tuguegarao City.
4. Upon physical examination, it was evident that Mr. Catuiran was positive of alcoholic
breath. He suffered a 3 cm lacerated wound at right frontal area as well as contusion
hematoma at the right periorbital area (more commonly known as black eye).
5. He also suffered soft tissue swelling at right occipital parietal area, contusion hematoma at
the left flank area and contusion hematoma at the right thigh. He also sustained abrasions on
the right and left knee and right and left elbow.
6. On that same day after examining Joshua Catuiran, I also performed a physical examination
on his companion Mr. Edward Singson, male, single and a resident of Gonzaga Ext., Ugac
Sur, Tuguegarao City.
7. Based on the examination I conducted Mr. Singson was positive of alcoholic breath. He
suffered from contusion hematoma at the left parieto orbital area, 3 cm lacerated wound at
right maxillary area and abrasions at right forearm, arm, leg and knee.
8. A 2x2 cm soft tissue swelling at left parieto temporal area and contusion hematoma was also
revealed during the examination.

I executed this affidavit to attest the truthfulness of the foregoing facts and for whatever
purpose it may serve
