Conclusion

 On detail investigation it stated that its been a captain’s negligence for a


non precision approach for landing because of the unsure decision made
by captain on the glideslope.
 The captain failed to prepare himself, the first officer, and the flight
engineer for the relatively complex localizer-only approach and also
failed to provide them with adequate guidance about monitoring the
approach; therefore, the captain’s approach briefing was inadequate.
 The first officer and flight engineer failed to properly monitor and
challenge the captain’s performance, which has led to the accident.
 Captain’s fatigue condition plays a prominent role in this accident.
 The Federal Aviation Administration’s Quality Assurance (QA) for the
Minimum Safe Altitude Warning System (MSAW) was inadequate at
Guam and the agency’s failure to adequately manage the system which
contributed to the flight 801 accident.

Recommendations
 UNSAFE ACTS
1. Captain fails to communicate with the crew members; which can be
enhanced by the better Cockpit Resource Management (CRM) training
program to develop all the required skills like interpersonal
communication skills, decision making skills, leadership skills etc.
2. RADAR failed to display as per Air Traffic Control (ATC) readings.

 PRE CONDITIONING FOR UNSAFE ACTS


1. High terrain and unclear glideslope incidents can be enhanced while
making better flight plan and focussing on weather and terrain details.
2. Implementing strict and severe medical check systems to minimise the
health problems such as pilot’s fatigue.

 UNSAFE SUPERVISION
1. Korean Air failed to provide effective training program for the flight
crew.
2. Implementing better CRM programs to generate better communication
since both the first officer and the flight engineer failed in correcting the
captain’s mistake while the approach.
3. Due to adequate resources; Korean Civil Aviation Bureau (KCAB) fails
to monitor the indicative safety problems.
4. In overall view, Federal Aviation Administration (FAA) awarded KCAB
as the First in ranking of safety despite few safety gaps.
 ORGANISATIONAL INFLUENCES
1. Implementing a survey regularly leads to monitor the hike and drop of the
employee’s performance and it makes the company to understand where
they should focus to improve.
2. Considering and Focussing on communication and recruiting pilot's
program; the pilot’s mistake such as CRM and fatigue respectively can be
minimised.
